I just spent about an hour searching, madchefs is quirky right now,and starchefs used to have forms... now its just a webpage with "normal" percentages for various establishments, Steakhouses etc... you could start your own spread sheet on excel? Not to sound silly but, instead of spending an hour online (I'm just bored and looking for something formal to give you)why is there any importance to a "form" . I have been explaining to my bookkeeper since the beginning of the year that the average cost per meal is $6.50 means absolutely nothing to me since we serve three meals with five different prices( brunch lunch breakfast picnic dinner cookout) sorry six prices. After seven months she finally understands that food cost isn't just purchases to revenue ( she wasn't subtracting my inventory and I kept getting a blank sheet at the end of the month) Any way the wine talks for me...

Depending on POS system used at your restaurant you'll be able to create your food cost analysis instantly to keep managers and owners happy. We use http://www.supperclubexpress.com and because it's an online POS and reservation system I can do my costing at home or anywhere there is a computer connected to the web. I usually have a hard time concentrating in the back office and most places I worked at before made me do excell sheets. I had to hand type data from the POS which was a waste of time because of double entry and errors while copy pasting. I DO adore the online capabilities of supper club express so DO check if your POS has something nifty like this, too.
