Most of those things are company spacific and need to be adjusted no matter what.

Q: What format are you looking for? Spreadsheet? Palm? Word? Is this ONLY for hard copy?

I may have a "hold harmless for food taken from site" form kicking around. Would that be of use? I will email it to you.

Alternately I also need updated versions of what you mention. Maybe we could trade work?
